From 1 July 2025, significant changes to Chronic Disease Management (CDM), as part of the Strengthening Medicare reforms, will come into effect. This 90-minute webinar will cover what is changing, why these reforms are being introduced, and how general practice can adapt to deliver better, more efficient care.
Other topics covered include the transition to a single Chronic Condition Management Plan, equalised MBS fees, simplified referral processes, and the removal of mandatory collaboration requirements. You will also learn about the role of MyMedicare in supporting continuity of care, and how this is a redesign – not a funding cut.
