Eacham Medical Centre is a friendly, busy, family medical centre, established 20 years ago. We have five GPs currently working with us. 

Our ideal candidate will be enthusiastic, highly organised, self motivated and have the ability to multi-task. They must have a passion for primary health, a positive attitude and be a strong team player. Training and further skill development available. 

Unconditional registration with AHPRA as an RN is essential.

Roles and responsibilities include:

  • immunisations (adult and children)
  • wound care
  • assisting doctors with procedures
  • triage and assessment of patients
  • treatment room procedures including ECG, spirometry, and ear syringing
  • chronic disease management - care plan development and reviews
  • assisting in the management of recalls and reminders
  • supporting and mentoring ENs, less experienced RNs and students as required.

The position is for covering maternity leave of another registered nurse for six-months, and ideally with flexibility to cover the leave of other nurses.

The position will commence mid-March 2025.

This is a rewarding opportunity for a passionate dementia professional to join the Dementia Support Australia Team at HammondCare on a fixed term full time contract until June 2026, based at Brisbane, Gold Coast, Rockhampton, Mackay or Townsville.

Responsibilities

  • Support families, caregivers, and clinical staff to understand how dementia impacts each individual.
  • Equip carers and clinicians to prevent and respond to behaviour changes through modelling, coaching, and development of resources including behaviour support planning.
  • Be able to synthesise information, through observation and written documentation to provide recommendations that are relationship-centred and informed by evidence based best practice.
  • Develop reports that are contextualised to individual situations and people, that effect change on the ground and improve quality of life for people living with dementia.
  • Undertake a range of assessments that help to understand the holistic experience of the person and their care network.
  • Work in partnership with the broader Dementia Support Australia to build capability across the home and aged care sectors.
  • Contribute to research, projects, and programs and work innovatively to meet the changing needs of the sector.
  • Work on other projects and assignments as required and in line with areas of skill, expertise and knowledge.

What do we need from you?

  • Expertise in supporting people living with dementia.
  • Experience in Australian residential aged care and or community aged care.
  • Be relatable and able to share knowledge in a way that supports change at all levels.
  • High level of negotiation, communication, and relationship building skills.
  • Excellent problem solving and decision-making skills.
  • Ability to prioritise multiple tasks and meet deadlines.
  • Excellent written and verbal communication skills.
  • Excellent stakeholder management.
  • A current Australian drivers licence and willingness to travel.
  • Empathy and patience.
  • Relevant qualification in allied health, nursing, aged care, or mental health is essential.
